Job summary

JOL Healthcare is seeking a Hospice Certified Nursing Assistant to provide compassionate personal healthcare in patients' homes under the direction of a Registered Nurse. You will assist with transfers, mobility, and nourishment while maintaining infection control and patient privacy.

The role requires strong communication with families and staff, accurate documentation, and flexibility to travel locally as needed to support hospice patients.

Qualifications

  • High school diploma preferred; completion of an approved aide program or state equivalent.
  • At least one year of hospice/home care experience preferred.
  • Ability to read, write, and verbally report clinical information to patients, representatives, caregivers, and staff.
  • Demonstrates genuine care for ill/elderly patients and a compassionate approach.

Job description

Overview:The primary function of the Hospice Certified Nursing Assistant (CNA) is to provide personal healthcare and related services to patients in their place of residence. The CNA assists in maintaining a safe and clean environment, works cooperatively with patients and families, and communicates observations and concerns to the supervising Registered Nurse (RN). All care is provided under the direction and supervision of an RN and in accordance with the Hospice Plan of Care.Essential Functions:Provides direct patient care under the direction of the RN and according to the Aide Plan of Care.Assists patients with safe transfers and ambulation per Hospice policy.Reports changes in patient condition and documents pertinent information and care provided.Practices accepted infection control principles and maintains a clean, safe, and comfortable environment.Promotes positive, supportive, and respectful communication with patients, families, and staff.Respects patient privacy and property.Assists with proper nutrition and adequate fluid intake.Contributes to the efficient operation of the Hospice and demonstrates effective time management.Performs treatments and procedures according to Hospice policy.Promotes the Hospice philosophy and adheres to administrative policies to ensure quality care.Job Qualifications:Education:High School Diploma preferred. Must have successfully completed one of the following:A) Training program/competency evaluation as specified in §418.76(b)(c) of the Medicare regulations;B) Competency evaluation program that meets §418.76(c);C) Nurse aide training/competency evaluation program approved by the state and listed in good standing on the state nurse aide registry;D) State Licensure program that meets requirements of §418.76(b)(c).Experience:At least one year of experience preferred as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility; or completion of a Certified Nursing Assistant or Certified Home Health Aide Program.Skills:Must be able to read, write, and verbally report clinical information to patients, representatives, caregivers, and hospice staff.Demonstrates interest in the welfare of the ill and elderly.Successful completion of the Aide Competency Evaluation Skills Checklist that meets §418.76(c) and written Skills Test.Environmental and Working Conditions:Works in patients’ homes under various conditions.Possible exposure to blood, bodily fluids, and infectious diseases.Must be able to work a flexible schedule and travel locally. Occasional exposure to unpleasant weather.Physical and Mental Effort:Prolonged standing and walking required.Must be able to lift up to 50 pounds and assist with patient mobility.Requires working under stressful conditions to meet deadlines, identify patient needs, make quick decisions, and address patient and family needs.Requires eye-hand coordination and manual dexterity. Ability to use durable medical equipment in the home.
