Job summary

AccentCare in Newark is seeking a Hospice Aide Coordinator to organize HA schedules with nursing leadership and provide mentorship for field staff. You will support training, oversee supplies, and assist with direct patient care per the interdisciplinary care plan.

The role requires at least 2 years in hospice care, relevant certifications, and strong communication skills. This is a full-time position with day shifts and no remote work.

Qualifications

• High school diploma required with current Nurse’s Aide, CHHA, or CGNA certification.
• CPR certification preferred or required in Georgia.
• At least 2 years of hospice care experience.
• CHAPNA certification preferred.
• Proficiency in Microsoft Office and EMR systems.
• Good written and verbal communication and ability to follow directions.

Responsibilities

• Coordinate and assign Hospice Aide schedules with nursing leadership.
• Mentor and liaison for HAs; support field training and orientation.
• Oversee medical supply management and cost control.
• Provide direct patient care as needed per the care plan.
• Collaborate with interdisciplinary team and families for coordinated care.
• May carry a patient caseload or support within other clinical licensure scope.
