Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under RN supervision. You will assist with transfers, communicate patient changes, and ensure documentation aligns with the Plan of Care.

The role requires an unencumbered CNA license and at least one year of relevant experience, with a focus on compassionate care and confidentiality. A mileage reimbursement program and company car may be available for qualifying individuals.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA license.
• At least one year of experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility.
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care.
• High integrity, including maintenance of confidential information.

Responsibilities

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures

What You Must Have:

• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
