Job summary

Holisticare Hospice is seeking a Per Diem Hospice Aide (CNA/HHA) to provide compassionate, hands-on end-of-life care to patients in Ocean County, NJ. Work with our nursing team to meet physical, emotional, and personal needs with dignity and respect. Schedule: Per Diem, weekends 7 am - 3:30 pm.

Qualifications

High School Diploma or GED required.
Valid HHA or CNA license required.
Minimum 6 months hospice or skilled nursing facility experience preferred.
Current driver's license and dependable transportation required.

Responsibilities

Provide personalized care following the care plan to ensure comfort and well-being.
Assist with personal care needs such as bathing, hygiene, and changing linens.
Deliver exceptional customer service and support for patients and families.
Support families with knowledge and guidance during end-of-life care.
Maintain accurate documentation in home and agency records.
