Job summary

Holisticare Hospice is seeking a Per Diem Hospice Aide (CNA/HHA) to provide hands-on care that brings comfort and dignity to patients at the end of life, working closely with our nursing team.

The role supports families, follows a care plan, and requires compassionate service; schedule evenings and weekends in Atlantic, Cape May & Cumberland counties. We offer a comprehensive benefits package and opportunities for growth.

Qualifications

• High School Diploma or G.E.D. required.
• Valid HHA or CNA license required.
• Minimum 6 months hospice or skilled nursing facility experience preferred.
• Current driver's license and reliable transportation required.

Responsibilities

• Provide Personalized Care: Follow the plan of care to ensure patient comfort and well-being.
• Assist with Personal Care: Help with bathing, hygiene, bedding, clothing, and grooming.
• Be Dedicated to Customer Service: Deliver compassionate, exceptional patient care.
• Provide Support for Families: Guide families with care during the terminal phase.
• Maintain Accurate Documentation: Record details per home health and agency standards.
