Job summary

Heart to Heart Hospice is hiring a CNA to deliver compassionate care under the supervision of a registered nurse. The role focuses on direct patient support, safe transfers, timely documentation, and infection control within a patient-centered team.

Ideal candidates hold an active CNA license, have a year of hospice/ Nursing Assistant experience, and demonstrate integrity, confidentiality, and reliable transportation to serve our community.

Qualifications

• High School Diploma preferred or GED.
• Unencumbered CNA certification per Nurse Aide Registry.
• Currently licensed as a CNA.
• At least 1 year as Hospice Aide or Nursing Assistant.
• Shows genuine care for ill and elderly, maintains confidentiality.
• EMR experience is a plus.
• Valid driver's license with reliable transportation.

Responsibilities

• Provide direct patient care under RN direction per Plan of Care.
• Assist with safe transfers and ambulation as per policy.
• Report and document changes to ensure continuity of care.
• Follow infection control principles.
• Communicate respectfully with patient, family and staff.
• Submit documentation promptly.
• Perform other duties as required.
• Ensure compliance with laws, Medicare rules and policies.
