Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under RN supervision. The role emphasizes compassionate care for ill and elderly patients, safe transfers, and thorough documentation to ensure continuity of care.

The position requires a valid CNA license, at least one year of Hospice Aide experience, and a drive for maintaining confidentiality and high integrity in all aspects of care.

Qualifications

• High School Diploma preferred or GED.
• Unencumbered CNA certification.
• Current CNA license.
• At least one year experience as Hospice Aide/Nursing Assistant.
• Interest in welfare of ill and elderly; compassionate care.
• High integrity, including confidentiality.
• EMR experience is a plus.
• Must have a valid driver’s license, reliable transportation, and ongoing auto liability insurance.

Responsibilities

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Assist the patient with safe transfers and ambulation per agency policy.
• Report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote respectful communication to patient/family and other employees.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and established policies.
