Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide/CNA to provide direct patient care under supervision, following the Plan of Care. You will assist with safe transfers, report changes, and document care while maintaining infection control standards and compassionate communication with patients and families.

Ideal candidates have a CNA license with at least one year of related experience, a valid driver’s license, and reliable transportation to support in-home and facility visits.

Qualifications

• High school diploma or GED preferred.
• Unencumbered CNA certification current.
• At least 1 year as Hospice Aide or Nursing Assistant in hospital, nursing home, home health, hospice or long-term care.
• Demonstrates compassionate care for ill and elderly; integrity and confidentiality.
• Must have valid driver’s license and reliable transportation; insurance.
• EMR experience is a plus.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist patient with safe transfers and ambulation per policy.
• Report and document care provided to ensure continuity of care.
• Practice infection control principles.
• Promote respectful communication with patient, family and staff.
• Timely submission of documentation.
• Perform other duties as required to ensure compliance with laws and policies.

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.

• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
