Job summary

Elara Caring is seeking a Hospice Aide - Certified Nurse Assistant for a full-time role in Michigan. You will provide skilled nursing care under a Plan of Care, monitor patients, and collaborate with the RN Case Manager to update care plans.

Education for patients and caregivers is a key part of the job. Qualified candidates have a High School Diploma or GED and at least 1 year of hospice aide or nursing assistant experience, plus a state-approved hospice training program.

Qualifications

• 1 year of Hospice Aide or Nursing Assistant experience required.
• State-established Hospice Aide training program meeting 42 CFR 484.36 and competency evaluation.
• Must be able to lift up to 50 pounds and assist with positioning/transfers.
• Valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Provide skilled nursing care per Plan of Care, orders, and RN direction.
• Monitor patient condition and report changes to RN Case Manager and care team.
• Collaborate with RN Case Manager to update the Plan of Care as needed.
• Educate patients and caregivers on self-care to support safety and independence.
• Document medication regimens and maintain accurate medication profiles.
• Help prevent avoidable hospitalizations through proactive, coordinated care.
• Provide compassionate, high-quality care with clear communication to patients and families.

Hospice Aide

• Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
• Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
• Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
• Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
• Document medication regimens and maintain accurate, up-to-date medication profiles.
• Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
• Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.

What is Required?

• High School Diploma or GED equivalent required
• 1 year of experience as a Hospice Aide or Nursing Assistant is required
• State-established or other Hospice Aide training program that meets the requirements of 42 CFR 484.36 and a competency evaluation program
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a home health environment is preferred
