Job summary

Elara Caring is seeking a Hospice Certified Nurse Assistant/Hospice Aide to provide compassionate, skilled care to patients in their homes. You will work under the supervision of the RN and collaborate with the care team to support safety, comfort, and quality of life.

Required: high school diploma or GED, 1 year hospice or nursing assistant experience, valid driver’s license or reliable transportation, and completion of a Hospice Aide program meeting 42 CFR standards; home health experience

Qualifications

• Must be able to lift up to 50 pounds and assist with patient transfers.
• Must have a valid driver’s license or reliable transportation to travel to patient homes.

Responsibilities

• Provide skilled nursing care per Plan of Care and RN direction.
• Monitor patient condition and report changes to RN Case Manager or care team.
• Collaborate with RN Case Manager to update the Plan of Care when needed.
• Educate patients and caregivers on self-care techniques for safety and independence.
• Document medication regimens and maintain updated profiles.
• Help prevent avoidable hospitalizations through proactive care coordination.
• Maintain compassionate, high-quality communication with patients, families, and care team.

Job Description

To continue to be an industry pioneer in delivering unparalleled care, we need a Hospice Certified Nurse Assistant/ Hospice Aide with commitment and compassion. Are you one of them?

• Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
• Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
• Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
• Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
• Document medication regimens and maintain accurate, up-to-date medication profiles.
• Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
• Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.

What is Required?

• High School Diploma or GED equivalent required
• 1 year of experience as a Hospice Aide or Nursing Assistant is required
• State-established or other Hospice Aide training program that meets the requirements of 42 CFR
• 484.36 and a competency evaluation program
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a home health environment is preferred
