Job summary

Elara Caring is seeking a Hospice Certified Nurse Assistant/Hospice Aide to join our team in Enid, OK. You will deliver skilled nursing care per the Plan of Care, report changes to the RN Case Manager, and collaborate with the care team to optimize patient outcomes.

Qualifications

• High School Diploma or GED equivalent required.
• 1 year of Hospice Aide or Nursing Assistant experience required.
• State-established Hospice Aide training program per 42 CFR 484.36 & competency evaluation.
• Ability to frequently lift, push, pull up to 50 pounds.
• Must have a valid driver's license or reliable transportation for travel to patient homes.

Responsibilities

• Provide skilled nursing care per Plan of Care and RN direction.
• Monitor patient condition and report changes to RN Case Manager and care team.
• Collaborate with RN Case Manager to update the Plan of Care as needed.
• Educate patients and caregivers on self-care techniques.
• Document medications and maintain accurate medication profiles.
• Help prevent avoidable hospitalizations through proactive patient care.
• Provide compassionate care with positive communication with patients and families.

Job Description

• Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
• Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
• Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
• Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
• Document medication regimens and maintain accurate, up-to-date medication profiles.
• Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
• Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.

What is Required?

• High School Diploma or GED equivalent required
• 1 year of experience as a Hospice Aide or Nursing Assistant is required
• State-established or other Hospice Aide training program that meets the requirements of 42 CFR
• 484.36 and a competency evaluation program
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a home health environment is preferred
