Job summary

Elara Caring is hiring a Hospice Aide to provide compassionate, in-home nursing support under physician orders and RN guidance. You will monitor patient status, update care plans, educate families, and document medications while maintaining safety and communication with the care team.

This full-time position requires a HS diploma, 1 year of hospice or nursing aide experience, and reliable transportation to travel between patients\' homes. Competitive benefits are offered.

Responsibilities

  • Provide skilled nursing care per Plan of Care and RN direction.
  • Monitor patient condition and report changes to the RN Case Manager and care team.
  • Collaborate with RN Case Manager to update Plan of Care as needed.
  • Educate patients and caregivers on self-care and safety.
  • Document medication regimens and maintain profiles.
  • Help prevent hospitalizations through proactive, coordinated care.
  • Maintain compassionate, high-quality care with clear communication.

Job Description: Hospice Aide Full-Time

To continue to be an industry pioneer in delivering unparalleled care, we need a Hospice Aide with commitment and compassion.

As a Hospice Aide, you’ll contribute to our success in the following ways:

  • Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
  • Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
  • Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
  • Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
  • Document medication regimens and maintain accurate, up-to-date medication profiles.
  • Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
  • Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.
What is Required?
  • High School Diploma or GED equivalent required
  • 1 year of experience as a Hospice Aide or Nursing Assistant is required
  • State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
  • Experience in a home health environment is preferred
