Qualifications

• Hold current unencumbered RN license.
• Bachelor's degree in nursing from an NLN-accredited program.
• One year of professional nursing experience in the last three years.
• Hospice/home health experience preferred but not required.
• CPR certification is current and valid.
• Valid driver's license and reliable transportation.

Responsibilities

• Review clinical referral information and respond to requests; collaborate on eligibility.
• Provide pre-assessment and admission services in patients' homes or facilities.
• Coordinate with Intake, referring physician, and Hospice Medical Director for referrals.
• Apply nursing principles and provide care, education, and emotional support.
• Participate in on-call duties as assigned.
