Trinity Health in Albany is seeking an experienced Hospice Intake & Admissions RN to join our central intake and admissions team. This full-time day shift role focuses on compassionate, patient-centered hospice care from the central intake office in Albany and field visits to patients’ homes and facilities across the region.

The RN will coordinate referrals, initiate care plans, and collaborate with a multidisciplinary team to ensure seamless transitions into hospice.

Qualificações

• Current license to practice as a Registered Nurse in New York
• Associates Degree in Nursing; a BSN is preferred
• 1 year of RN experience required
• Home care or hospice experience is a plus
• Valid driver’s license and reliable transportation
• Excellent communication and concise documentation abilities

Responsabilidades

• Serve as a key liaison between patients, families, providers, and the interdisciplinary team
• Review and coordinate referrals to determine hospice eligibility with physicians and care teams
• Meet patients where they are, at home or in care facilities, to complete initial assessments
• Initiate care plans, including ordering medications, equipment, and supportive services
• Partner with social workers, chaplains, aides, therapists, and volunteers for holistic care
• Ensure smooth handoffs to the Hospice Case Manager
