The Community Hospice is seeking a compassionate and motivated Registered Nurse (RN) to join our Weekend Hospice Admissions Team. This rewarding role focuses primarily on admissions to our Hospice Home Care program, with cross-training opportunities to support admissions across other levels of hospice care.
The Hospice Admissions RN is responsible for evaluating and admitting patients to hospice services in the home setting while ensuring a smooth transition into care. This role combines clinical expertise, compassionate communication, and coordination with physicians, patients, families, and the hospice interdisciplinary team.
The Community Hospice's corporate office is located at 445 New Karner Road, Albany, NY, with satellite offices in Catskill and Saratoga Springs, NY. Admissions are completed throughout our eight-county service area.
Training provided in end of life care.
Community Hospice provides a thorough orientation, preceptors and a supportive work environment.
Experience based pay range for NON-weekend track: $38.00-$50.18
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
