The Community Hospice is seeking a Per Diem Hospice Registered Nurse to join the Central Intake & Admissions Team in Albany. As the Intake/Admissions RN, you’ll be the first contact on a patient’s hospice journey, delivering compassion, coordinating referrals, and guiding families through transitions to hospice care.

This part-time, day-shift role requires an active New York RN license, ASN (BSN preferred), and home care experience.

Qualifications

• Active New York State RN license required.
• ASN required; BSN preferred.
• Home care or hospice experience strongly preferred; end-of-life care exposure a plus.

Responsabilités

• Serve as liaison between patients, families, providers, and the care team.
• Review referrals to determine hospice eligibility with physicians and care teams.
• Complete initial assessments at patients' homes or care facilities.
• Initiate care plans, including orders for medications, equipment, and services.
• Collaborate with social workers, chaplains, aides, therapists, and volunteers.
• Ensure smooth handoff to the Hospice Case Manager.
