Job summary

Tufts Medicine Care at Home is seeking a Hospice Admissions Nurse to work full-time at MelroseWakefield Hospital, coordinating pre-admission activities and admission decisions for hospice-appropriate patients. You will verify insurance, explain benefits, and document assessments in the EMR while collaborating with a multidisciplinary team.

The role requires an active RN license in the state, hospice or end-of-life experience, and CPR/BLS certification.

Qualifications

• Active RN license in the state where services are provided.
• Hospice and/or end-of-life experience required.
• CPR certification or BLS certification within required timeframe.

Responsibilities

• Explain hospice services to patients and families and address questions.
• Verify insurance eligibility and document in EMR.
• Explain insurance coverage and financial obligations and escalate when needed.
• Promote hospice benefits and communicate with referral sources.
• Coordinate admission packets and obtain signed NOE and forms.
• Communicate assessment findings to the hospice medical team and ensure handoff.
