Job summary

Good Shepherd Community Care seeks an Admissions Nurse Liaison to join its expanding Intake and Admissions team in Greater Boston. Travel is required as services are provided in patients’ homes and facilities throughout the region.

The RN will assess patients, explain hospice services, and coordinate smooth admissions with hospital staff and referral sources. MA licensure and 2+ years in relevant fields are preferred.

Qualifications

• Licensed RN eligible to practice in Massachusetts.
• Two years of experience in community health, oncology, hospice, or palliative care preferred.
• Strong communication and interpersonal skills; ability to coordinate admissions.

Responsibilities

• Explain hospice services to patients, families, and referral sources.
• Conduct clinical assessments and coordinate admissions.
• Maintain professional relationships with referral sources and Good Shepherd staff.
• Travel to patient homes and facilities in Greater Boston and Metrowest.
