Qualifications

• Coordinates care as Case Manager when assigned by Clinical Supervisor.

Responsibilities

• Completes comprehensive patient and family assessments to determine hospice needs.
• Provides professional nursing care per the physician's plan of care.
• Writes and initiates plans of care and revises them as patient needs change.
• Communicates with physicians and care team to coordinate plans and education.
• Ensures safe environment and continuity of care for patients and families.
