Job summary

Peoples Health Services seeks a Hospice Admissions Nurse to perform hospice consultations and admission visits, assessing patient and family needs. You will collaborate with the attending physician and hospice physician to determine clinical eligibility and complete admission documentation for Peoples Hospice.

The role requires a Florida or multi-state RN license, CPR certification, and availability for evenings, nights, weekends, and holidays.

Qualifications

• Current Florida or multi-state RN license required (active).
• Two years of RN experience preferred.
• Experience in acute, oncology, home health, and/or hospice setting preferred.
• Availability to work evenings, nights, weekends, and holidays.
• Current CPR certification is required.

Responsibilities

• Performs consultation/admission visits to assess patient and family needs and determines hospice eligibility.
• Completes admission assessment and Hospice Election of Benefit documentation.
• Maintains accurate records and submits reports on time.
• Coordinates with attending physicians and IDG to plan patient care.
• Manages a caseload of four to five patients and develops Care Plans with the IDG.
