Job summary

CompassionCare Hospice in Las Vegas, NV is seeking a dedicated Hospice Admissions RN to join our team. You will visit patients referred for hospice care and assess eligibility, coordinating services with families and the interdisciplinary team.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, ICU, or Emergency Room will be considered in place of hospice nursing experience.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Visit patients referred for hospice care in homes, hospitals, nursing facilities, etc.
• Explain hospice services, discuss goals and care with the patient and families, answer questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
