Job summary

CompassionCare Hospice, an affiliate of BrightSpring Health Services, seeks a Hospice Admissions RN in Las Vegas. You will visit patients, assess eligibility, coordinate start of services, and collaborate with an interdisciplinary team to deliver 1:1 patient-centered care.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in NV.
• Minimum 1 year hospice nursing experience; 3+ years in related settings accepted.
• Valid driver’s license and insured vehicle per state requirements.

Responsibilities

• Visit patients in homes, hospitals, or facilities who are referred for hospice care.
• Explain hospice services, discuss goals and care with patients and families, and obtain necessary consents.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care


• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents


• Obtain medical history, current medications, and symptoms


• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines


• Coordinate the start of services (nursing, aides, equipment, medications.)


• Collaborate with interdisciplinary team


• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation


• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience


• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
