Job summary

Hospice of North Alabama is seeking a dedicated Hospice Admissions RN to join our team in Decatur, AL. The role involves visiting patients in homes and facilities, explaining hospice options, and coordinating start of services with an interdisciplinary team.

The ideal candidate is a graduate of an accredited nursing program with active licensure, at least 1 year of hospice nursing experience (3+ years in related settings welcomed), and a valid driver’s license.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, ICU, or ER will be considered.

Responsibilities

• Visit patients (homes, hospitals, nursing facilities) referred for hospice care.
• Explain hospice services, discuss goals and care with patients and families, obtain consents and legal documents.
• Obtain medical history, current medications and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice per Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
