Hospice of North Alabama is seeking a dedicated Hospice Admissions RN to join our team in Decatur, AL. The successful candidate will provide patient-centered hospice care in diverse settings, perform thorough assessments, and coordinate start-up of services with the interdisciplinary team.

This role emphasizes compassionate leadership, accurate documentation, and collaboration with families, patients, and healthcare partners to ensure quality end-of-life care.

Qualificações

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year hospice nursing experience required; 3+ years in LTC/ICU/ER acceptable in lieu.
• Must have a valid driver's license and insured vehicle.
• CPR certification is required in states where applicable.

Responsabilidades

• Visit patients in homes, hospitals, nursing facilities, and other settings for hospice care.
• Explain hospice services, discuss goals and care with patients and families, obtain consents and documents.
• Obtain medical history, current medications and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice per Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with the interdisciplinary team and maintain timely documentation.
