Qualifications

Graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
Current nursing licensure in State
Master's degree with minimum 1 year NP experience and 1 year hospice/palliative care experience

Responsibilities

Provide medical care and manage terminal illness under supervision of a collaborating physician
Complete history and physicals and document outcomes
Develop individualized hospice plans, prescribe medications and coordinate care
Meet hospice face-to-face visit requirements for eligibility
Communicate with hospice medical director on eligibility and recertification
Review and coordinate patient care with the interdisciplinary team
Ensure symptom control through team collaboration
Develop and revise care plans with guidance from physicians
Fulfill assignments and support end-of-life counseling
Other duties as assigned
