Qualifications

• Current RN license in the applicable state(s).
• Minimum of 1 year clinical RN experience.
• CPR certification required.

Responsibilities

• Coordinate admissions of hospice patients with initial assessments.
• Establish initial care plan with physicians and interdisciplinary team.
• Provide skilled interventions to support symptom control and comfort.
• Serve as liaison among disciplines during admission process.
• Maintain individualized patient care progress records and documentation.
