Qualifications

Responsibilities

• Coordinates admissions of hospice patients with initial physical, psychosocial, and spiritual assessments.
• Establishes initial plan of care with physicians and interdisciplinary team.
• Provides skilled interventions to ensure symptom control and comfort.
• Acts as liaison between disciplines and facilities during admissions.
• Maintains patient care progress records with accuracy and timely documentation.
• Communicates changes in patient condition to the team and physicians.
• Performs on-call duties and other assigned tasks.
