Qualifications

• Active RN license in the state of employment (or eligible)
• 1+ year of general nursing experience
• Compassionate, empathetic, patient-centered approach
• Strong assessment, communication, and interpersonal skills
• Willingness to visit patients in homes, hospitals, or facilities
• Current CPR certification

Responsibilities

• Serve as ambassador for hospice services and report to Admissions Manager
• Visit patients and families to assess eligibility for hospice care
• Explain hospice philosophy clearly and compassionately
• Guide admissions to ensure a smooth start to care
• Coordinate with physicians, case managers, and facilities
• Document findings and ensure regulatory compliance
