Job summary

James River Hospice is actively seeking a compassionate Admissions/Resource Registered Nurse (RN) to join our growing team in Virginia. You will evaluate hospice eligibility, coordinate admissions, and help patients and families transition smoothly into hospice care.

You will also provide compassionate care to General Inpatient Hospice (GIP) patients in hospital settings and support hospice patients in the community as needed. Schedule: Monday - Thursday, 9:00 AM - 7:00 PM.

Qualifications

• Must hold a valid Virginia Registered Nurse license.
• Experience with hospice admissions and care planning is preferred.
• Graduate of an accredited nursing program with active licensure.
• Must have a valid drivers license and reliable vehicle.

Responsibilities

• Evaluate patients for hospice eligibility and coordinate admissions.
• Provide GIP and community-based hospice care as needed.
• Develop individualized care plans and educate families on hospice services.
