Residential Home Health and Hospice is seeking an Admissions Registered Nurse (RN) to evaluate and admit patients into hospice care in Lisle, IL. This role requires driving to patient homes, managing assessments, and coordinating with a multidisciplinary team.

Qualified candidates will have an active RN license, at least two years of RN experience, and the ability to travel roughly 20% of the workday. A $9,000 first-year bonus is offered and benefits include medical, dental, vision, and more.

Responsibilities

• Deliver compassionate, patient-centered hospice admission care
• Conduct comprehensive hospice admission assessments and develop plans of care
• Educate patients and families on the admission process and hospice philosophy
• Perform accurate EMR documentation and coordinate with interdisciplinary team
• Coordinate evaluations with other disciplines as needed
