Job summary

Anchor Health is seeking an Admissions Registered Nurse in California to present our services to patients and families and drive appropriate admissions. The role reports to the Admissions Manager and requires RN licensure in CA, CPR certification, and strong communication skills.

Qualifications

• Graduate of a National League of Nursing accredited school of nursing.
• Current licensure in CA and CPR certification.

Responsibilities

• Admissions RN participates in information gathering, analysis, education, and form completion of admissions.
• Evaluates patients for hospice appropriateness through medical records review.
• Obtains agreement for admission of referred patients meeting criteria.
• Analyzes data to facilitate admission orders.
• Builds relationships with referral sources to ensure high-quality responsiveness.
• Maintains clinical and professional competence.
• Develops internal relationships with admissions and patient care staff.
• Educates patients and families about hospice and palliative care.
