Job summary

Serenity Hospice Care in New Jersey seeks an experienced Admissions Registered Nurse (RN) to evaluate patients for hospice eligibility, complete admission assessments, and coordinate the start of care. You will educate patients and families about hospice services and collaborate with the interdisciplinary team to ensure a smooth transition to hospice care.

Qualifications

• Current, unrestricted RN license in the state of practice.
• Minimum of one year hospice nursing experience.
• Strong assessment and critical thinking skills.
• Excellent verbal and written communication abilities.
• Ability to work independently while managing multiple priorities.
• Valid driver's license and reliable transportation.
• Proficiency with EMR systems preferred.

Responsibilities

• Conduct comprehensive hospice admission assessments in patients' homes, facilities, or hospitals.
• Determine hospice eligibility in collaboration with the hospice physician and interdisciplinary team.
• Complete all required admission documentation accurately and in a timely manner.
• Develop and initiate individualized plans of care.
• Educate patients and families on hospice philosophy, services, medications, and expectations.
• Coordinate admissions with referral sources, physicians, hospitals, nursing facilities, and case managers.
• Obtain informed consents and ensure all regulatory requirements are met.
• Collaborate with clinical staff to facilitate seamless transitions of care.
• Maintain compliance with Medicare, state, and agency regulations.
• Participate in on-call or weekend rotation as assigned (if applicable).
• Provide compassionate emotional support to patients and families during the admission process.
