Qualifications

• Active Massachusetts RN license required.
• 1–2 years nursing experience; hospice, palliative care, or home health preferred.
• Strong clinical assessment and symptom management skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct in‑home assessments to determine hospice eligibility per Medicare guidelines.
• Complete admissions including clinical documentation, consents, and care planning.
• Collaborate with physicians, case managers, and team members to facilitate timely admissions.
• Educate patients and families on hospice philosophy, services, and expectations.
• Develop and initiate individualized plans of care.
• Document accurately in the EMR/health record.
• Coordinate DME, medications, and initial services.
• Provide emotional support to patients and caregivers during admission.
• Maintain compliance with regulatory and agency standards.
• Participate in on‑call rotation as required.
