  • Proficiency with EMR systems preferred.

Responsibilities

  • Conduct comprehensive hospice admission assessments in patients' homes, facilities, or hospitals.
  • Determine hospice eligibility in collaboration with the hospice physician and interdisciplinary team.
  • Complete all required admission documentation accurately and in a timely manner.
  • Develop and initiate individualized plans of care.
  • Educate patients and families on hospice philosophy, services, medications, and expectations.
  • Coordinate admissions with referral sources, physicians, hospitals, nursing facilities, and case managers.
  • Obtain informed consents and ensure all regulatory requirements are met.
  • Collaborate with clinical staff to facilitate seamless transitions of care.
  • Maintain compliance with Medicare, state, and agency regulations.
  • Participate in on-call or weekend rotation as assigned (if applicable).
  • Provide compassionate emotional support to patients and families during the admission process.

Position Summary

We are seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our hospice team. The Admissions RN is responsible for evaluating patients for hospice eligibility, completing admission assessments, coordinating the start of care, and educating patients and families about hospice services. This role requires excellent clinical judgment, strong communication skills, and the ability to work collaboratively with interdisciplinary team members to ensure a smooth transition into hospice care.
