James River Hospice is seeking a compassionate Hospice Admissions/Resource RN to join our growing team in Virginia. You will evaluate patients for hospice eligibility, coordinate admissions, and guide families through transition to hospice care, while also supporting GIP patients in hospital settings.

Schedule: Monday–Thursday, 9:00 AM–7:00 PM. The role includes travel, competitive benefits, and opportunities for career advancement within a locally owned company.

Qualificações

• Must be a Registered Nurse with a Virginia license.
• At least 1 year of hospice experience is required.
• Graduate of an accredited nursing program.
• Experience with hospice admissions, patient assessments, and care planning preferred.
• Must have a valid driver’s license and a reliable car.

Responsabilidades

• Complete hospice admissions and evaluate eligibility for hospice services.
• Coordinate care to ensure smooth transition into hospice.
• Educate patients and families on hospice services and end-of-life care.
• Provide care to GIP hospice patients in hospital settings and support community hospice team with visits, pain and symptom management and other clinical needs.
