Amedisys Home Health is seeking an experienced admissions registered nurse for hospice services in East Hartford, CT. This full-time role involves coordinating admissions, assessing patient needs, and ensuring a smooth care plan with physicians and the care team.

Shifts are 11-7 or 12-8, Monday through Friday, with annual pay between 92k and 100k. The position emphasizes compassionate home-based care and collaboration across disciplines to improve patient comfort and outcomes.

Qualifications

• Active RN license required in the state(s) of assignment.
• At least 1 year of clinical RN experience.
• Current CPR certification required.

Responsibilities

• Coordinates admissions of hospice patients with initial assessments (physical, psychosocial, spiritual).
• Establishes initial care plan with physician and interdisciplinary team.
• Provides skilled interventions and caregiver instructions to optimize symptom control and comfort.
• Acts as liaison between disciplines and facilities during admission process.
• Maintains individualized care progress records and submits documentation per policy.
• Communicates findings and changes to clinical manager/physician and hospice team; reports patient needs and orders as required.
• Performs on-call responsibilities and on-call services as assigned.
• Performs other duties as assigned.
