Job summary

James River Hospice is seeking a compassionate Hospice RN Case Manager to oversee intermittent hospice case management in patients' homes. You will coordinate with the interdisciplinary team, supervise LPNs and aides, and ensure comfort and quality of life through hands-on pain management.

Qualifications

• Must be a licensed RN in Virginia.
• At least 1 year of hospice experience preferred.
• Valid driver's license and reliable car required.

Responsibilities

• Initial patient and family assessment to determine hospice needs.
• Develop an individualized plan of care focused on comfort and quality of life.
• Coordinate with interdisciplinary team and supervise LPNs and aides.
• Provide hands-on pain and symptom management; adjust care plan as needed.
• Participate in on-call rotation as required by policy.
