Job summary

Aspire Talent Solutions is seeking a Hospice Admissions RN in the Kansas City Metro, MO (east) for a full-time on-site role. The nurse will coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and work with the interdisciplinary team to ensure a smooth transition into hospice services.

Requirements include Missouri RN license, current CPR, driver's license, two years of RN experience in a clinical setting, and one year of hospice experience.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current driver's license.
• Current CPR certification.
• Two years of RN experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, & interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.
