Job summary

True North Consultants, Inc. seeks a full-time Hospice Admissions RN in the Kansas City Metro (east), MO.

This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner with the interdisciplinary care team to ensure a smooth transition into hospice services. You will perform head-to-toe assessments, ensure timely documentation, and support ongoing communication with care teams.

Qualifications

• Current RN license valid for practice in Missouri.
• Current driver's license.
• Current CPR certification.
• Two years of RN experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.
