Job summary

Trinity Health Of New England At Home is seeking a Hospice Admissions Registered Nurse (RN) to provide skilled nursing care in patients' homes across Waterbury region and surrounding areas. This full-time role offers a 4-day work week, day shift (11 am–8 pm on Mon/Tue/Thu/Fri), and a $10,000 sign-on bonus.

You will develop individualized care plans, administer medications, educate patients and families, and support recovery and independence while delivering compassionate 1:1 care.

Qualifications

• Graduate of an approved nursing education program; BSN preferred.
• Current Connecticut RN license.
• 1 year of professional acute care nursing experience.
• Home care/hospice experience preferred but not required.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and monitor patient status.
• Educate patients and families and support recovery and independence.
• Coordinate care and protect patient comfort and safety.

Requirements:

• Graduate of an approved nursing education program, BSN preferred.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse
• Home care/Hospice experience preferred but not required
• Must have current Driver's license and reliable transportation
