Job summary

Trinity Health Of New England At Home in Hartford seeks a Hospice RN Case Manager (RN) to deliver skilled nursing care in patients' homes, develop individualized care plans, administer medications, and educate patients and families.

This full-time role offers a Monday–Friday schedule with occasional back-up for holidays or weekends, a $10,000 sign-on bonus, and opportunities for growth within a nurse-led home health team.

Qualifications

• Graduate of an approved nursing education program.
• One (1) year experience as a professional acute care nurse.

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and educate patients and families.
• Support recovery and independence through patient-centered care.
• Build 1:1 patient relationships and promote continuity of care.

Requirements:

• Graduate of an approved nursing education program, BSN preferred.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse
• Home care experience preferred but not required
• Must have current Driver’s license and reliable transportation
