Job summary

Trinity Health Of New England At Home (Connecticut) is seeking an experienced Home Health Registered Nurse Case Manager in the Hartford region. The role focuses on delivering skilled nursing care in patients' homes, developing personalized care plans, administering medications, and educating patients and families to support recovery and independence.

The position offers a Mon–Fri 8 am–5 pm schedule with occasional backup for holidays or weekends.

Qualifications

• Graduate of an approved nursing education program, BSN preferred.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse.
• Home care experience preferred but not required.
• Must have current Driver’s license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and educate patients and families.
• Support recovery and independence with compassionate, one-to-one care.
• Provide care consistent with home health standards and physician orders.

Hospice RN Case Manager (RN)

Location: Hartford Region

Status: Mon - Fri 8 am to 5 pm, occasional backup for holiday or weekend shift

As a Home Health Registered Nurse, you’ll deliver skilled nursing care in the comfort of patients’ homes, creating individualized care plans, administering medications, educating patients and families and supporting recovery and independence.
