Job summary

Trinity Health Of New England At Home is seeking a Home Health Palliative Care RN in the Enfield region. The role requires CT RN licensure and a BSN preferred, with a 8 am–5 pm schedule, Mon–Fri and rotation every 4 weeks for weekends and holidays.

Home Health Palliative Care RN

Location: Enfield Region

Status: Mon – Fri 8 am to 5 pm, weekend rotation every 4 weeks, 2 holidays

The Palliative Care RN is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation. Provides primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization’s policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. Provides functional support/supervision to Home Healthcare Aides and LPNs as needed. Provides therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction.

Requirements:

• Graduate of an approved nursing education program, BSN preferred.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse
• Home care experience preferred but not required
• Must have current Driver’s license and reliable transportation
