Job summary

Compassus in Santa Fe, NM is seeking a Hospice Admission Registered Nurse to assess eligibility for hospice services, explain care options, and coordinate with the care team. This role emphasizes compassionate, patient- and family-centered care in a home-based setting.

The ideal candidate holds an RN license in the state, has hospice experience, and demonstrates strong collaboration with interdisciplinary teams to address patient needs from day one.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse, hospice experience preferred.
• Current CPR and BCLS certification.
• Valid driver's license is required.

Responsibilities

• Assess patients for hospice eligibility and explain hospice care services to patients and families.
• Collaborate with the Inter-Disciplinary Team regarding initial plan of care and patient condition.
• Ensure all patient initial needs are addressed.
• Support patients and families to spend quality time with loved ones while receiving care at home.
