Job summary

Compassus is seeking a Hospice Admission Registered Nurse (RN) to join our Suffolk, VA team. You will assess patients for hospice eligibility, explain services to families, and coordinate with the interdisciplinary team to plan care that supports quality time at home.

Qualifications

• RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Current driver’s license.
• Current CPR and BCLS certification.

Responsibilities

• Enable patients to spend quality time with loved ones at home.
• Assess patients for hospice eligibility and explain services.
• Explain hospice care to patients, families, and caregivers.
• Ensure initial patient needs are addressed.
• Collaborate with the Inter-Disciplinary Team on the care plan.
