Job summary

Reliant at Home in Plano, TX is seeking a Full-Time Hospice Clinical Liaison. The role blends clinical nursing with relationship-building to assess hospice eligibility, admit patients timely, and partner with referral sources.

As admissions nurse and community liaison, you will educate families about hospice services, coordinate with the care team, and expand partnerships with hospitals, SNFs, and physicians. RN license required with hospice experience preferred.

Qualifications

• RN license in good standing.
• Graduate of an accredited nursing program (Associate's or Bachelor's).
• Minimum of 1 year of hospice, palliative care, or related healthcare experience.
• Previous experience as a clinical liaison, admissions, or healthcare sales preferred.
• Strong assessment skills and familiarity with hospice eligibility criteria.

Responsibilities

• Conduct timely clinical assessments to determine hospice eligibility.
• Collaborate with the interdisciplinary team to plan patient care.
• Educate patients and families on hospice services and expectations.
• Ensure admissions documentation is accurate and compliant.
• Maintain knowledge of hospice guidelines, insurance, and community resources.
• Provide emotional support to patients and families during transition to hospice.
• Build relationships with hospitals, SNFs, physicians, and referral partners.
• Promote hospice services through education, presentations, and outreach.
• Identify referral opportunities and coordinate with intake and clinical teams.
• Represent the organization at health fairs and referral meetings.
• Track referral patterns and provide leadership with insights.
