The Hospice Liaison/Admissions Nurse at Atlantic Health coordinates admissions from hospitals, facilities, physicians and community sources, guiding patients and families through the transition to hospice services.

Working with discharge planners, physicians and hospice leadership, this role ensures regulatory and reimbursement compliance, delivers education to referral sources, and maintains strong partnerships to support timely, compassionate admissions.

Qualifications

• Clinical knowledge of hospice services and eligibility.
• Ability to educate referral sources and families about levels of care.
• Strong communication and customer service orientation.

Responsibilities

• Develop and coordinate the initial plan of care for hospice patients.
• Collaborate with patients, families, physicians, and discharge planners for admission information.
• Assess eligibility and coordinate appropriate level-of-care placement.
• Consult with hospice management on General Inpatient admissions as needed.
• Educate patients and families about hospice philosophy, services, benefits, and levels of care.
• Coordinate admission orders and certify terminal illness with Hospice Medical Directors.
• Facilitate timely admissions while ensuring a positive patient/family experience.
• Maintain knowledge of regulations, reimbursement guidelines, and industry best practices.
• Build relationships with referral sources and community partners.
• Participate in quality improvement initiatives.
