Elara Caring is seeking an experienced Hospice Admissions Nurse in Lansing, MI to join our dedicated home-based care team. You will assess patients, complete initial evaluations, and coordinate care under a Plan of Care to ensure quality and timely support.

This full-time role requires an active RN license, 1+ year of clinical nursing, and the ability to lift up to 50 pounds. Driving to patient homes is expected; HomecareHomeBase experience is a plus.

Qualifikationen

• Current, unrestricted RN license in the state.
• Graduate of an accredited nursing program (certificate/associate/bachelor).
• At least 1 year of clinical nursing experience.
• Ability to lift and transfer patients up to 50 pounds.
• Must have a valid driver’s license or reliable transportation.
• Associates or Bachelors in Nursing preferred.
• Previous Home Health or Hospice experience preferred.
• HomecareHomeBase experience preferred.

Aufgaben

• Assess hospice patients to identify physical, psychosocial, environmental, and nursing needs.
• Complete initial evaluations and reassess patient needs during visits.
• Provide care per Plan of Care to support quality and continuity of care, and positive patient outcomes.
• Monitor for changes in patient condition and adjust the Plan of Care as needed.
• Communicate significant changes, concerns, or safety issues to the physician and care team.
• Maintain accurate clinical documentation while protecting patient privacy per HIPAA requirements.
