Job summary

Elara Caring in Michigan is seeking a Licensed Practical Nurse to join our hospice/home health team, delivering compassionate care in patients' homes.

You will follow individualized care plans, monitor patient progress, educate families, document medications, and collaborate with the RN Case Manager and physicians to adjust plans as needed. An active LPN license and at least 1 year of clinical experience are required.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse (LPN)
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, or have reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

Education

Licensed Practical Nurse license (LPN)

Graduate of accredited nursing program (Certificate/Diploma/Associate)
