Goodwin Living in Virginia is seeking an Admission Nurse to oversee hospice admissions. The RN will conduct admission assessments, identify patient needs, and devise a Plan of Care under the physician’s order to ensure optimal functioning.

You will present hospice services to patients and families, coordinate with the interdisciplinary team, document care plans, and support discharge planning, while maintaining confidentiality and complying with safety standards.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed to work as a Registered Nurse in the Commonwealth of Virginia.
• CPR certification.
• Ability to communicate effectively in English, both verbally and written.

Responsibilities

• Presents hospice services and philosophy of care to patients and families.
• Performs admission assessments and determines eligibility for hospice.
• Develops Plan of Care under physician orders to address patient needs.
• Coordinates with interdisciplinary team and documents plans of care.
• Prepares for discharge planning and maintains timely documentation.
• Maintains confidentiality and follows infection control and safety measures.
